GRANT SUMMARY
The City Manager applied for and on September 26, 2023, the City was awarded grant funds in the amount of $67,135 to the Department of Library and Recreation Services for the implementation of disease prevention and health promotion programs. The grant will fund casual staff to coordinate the program, training for new evidenced-based programs, purchase of two laptops to support current Sunnyvale programs, and efforts to seek new agencies that serve older adults in Santa Clara County to implement evidence-based health programs to under-served older adults in their community.
Evidence-based programs are grounded in the rigorous study of interventions have been proven to produce positive changes or outcomes for participants. This grant will implement the following evidence-based programs: SAIL (Stay Active and Independent for Life), Tai-Ji Quan, Moving for Better Balance, and Bingocize. The grant supports relationship building, training, and technical support. It will also fund program implementation during the one-year grant period from October 1, 2023, to September 30, 2024.
Granting Agency
The County of Santa Clara's Area Agency on Aging, Sourcewise, a 501(c)(3) nonprofit organization, with a mission to provide adults and their caregivers the tools and services they need to effectively navigate their health and life options.
The Sourcewise Title III D Disease Prevention and Health Promotion ARPA grant funds have federal reporting requirements.
